6. Injury Report

• Player Injury Report - fill out if player has been injured during practice or games.

5. Injury Claim Form

Should your player sustain injuries and had out of pocket injuries because of this injury, you may be able to recoup some monies.

Your team manager must have filled out the online Player Injury Report for you to proceed.

Print out a copy of this Accidental Injury Claim Form - last revised 2017 (pdf) and follow the instructions.  Injury claims must be completed within 30 days of injury.
